When you choose Sierra Home Health Care Services, you can rest assured that you or your loved ones are receiving the highest quality care from qualified, compassionate, and personable caregivers. To ensure that they meet these stringent standards, we require all new employees to undergo both entry-level and monthly training. This training includes topics such as infection control, skin care, and client communication.
Every patient that enrolls to receive home health services through Sierra Home Health Care Services undergoes an initial assessment with a Registered Nurse to determine the care they require. During this initial assessment, we may inquire about medical history, home environment, preferences, and concerns. We use this information to develop a personalized Plan of Care that fits the unique needs of each patient. All patients are reassessed on a periodic basis to ensure that they are receiving the care they need. In the event that the patient’s needs suddenly change, a reassessment may be performed in as soon as 24 hours, if appropriate.
We recognize that our clients come from diverse backgrounds and may not be comfortable speaking English. For such clients, we aim to send caregivers that are able to speak their native language. We currently have caregivers that are fluent in English, Spanish, Korean, Japanese, Mandarin, Cantonese, Tagalog, and Persian. For clients with sensory impairment, we offer sign language interpreters and other auxiliary aids.
